Two things I find most helpful: Blocking the Internet, and timers.
The first may not be very practical for you during work hours, unless you do your surfing on another computer/device. But might be helpful for a couple of hours in the evenings and weekends.
If I find myself going into a frequent haze (Internet or otherwise), I start setting timers or reminders to knock me out of that haze every XX minutes, or to remind me to move on to the next task, or whatever.
I have been trying to be a bit less of a control freak lately (part of an initiative to feel less like a word factory and more like a creative human being), but I definitely need my Internet blocker to keep me from numbing out on the Internet instead of being creative. (Yes, it did requires some changing of my tools and workflow. It was worth it.)
